Clutch Problem

I have a strange problem with my clutch. I believe it is an Exedy twin disc. In the past it has never slipped...today, I was pulling out from a stop sign and it kinda popped out of first gear and then after that the clutch started slipping. Any ideas?

Update:

So driving home from work, I was easy on the car....once I got on the highway I did pulls in 3rd, 4th and 5th from about 30mph.....clutch did not seem to slip at all...

when i got on the car earlier, could i have heated the clutch up that fast to make it slip?
