I've actually seen this two times. I have no idea why it happens but it's a problem.

Idk. I think they just vibrated out. There is no broken off bolt in the threads or anything. On the first alternator I replaced I saw two of them loose, almost all the way out and tightened them up. It went out a month later. Its obviously a proplem. Im tired of replacing it. heh. Its not easy

So i guess the company making the alternators is skimping on locktite.
