Why my serpentine belt tore out?

Why my serpentine belt tore out?
Never seen this before on any car. What could have cause this?
Right before it tore out I could hear a high pitch noise over 2800RPM, that sounded like the belt was slipping. A couple hours later the noise changed and it was tore out.
I thought the tensioner whas the culprit and changed it along a new belt. No change. At idle I still can hear a random chirp.
All accessories turn freely by hand. The alternator is the - much - heavier one. The crank pulley is a Fluidampr one.

My friend had the same problem after using an aftermarket alternator on his 03 evo 8. It turns out the belt did not sit correctly on the pulley and would shred the belt. We changed the pulley to a mitsubishi oem pulley and resolved that problem. Your issue may be similar.

I had a weird issue years ago and the belt snapped on me at the track. My car vibrates more than others, why I don't know. Overtime the starter bracket (I think could be alternator bracket) broke and nobody noticed it. This caused the belt to be moved in a odd slight angle. This angle overtime put more pressure on one side of the belt. It snapped. After a 2nd belt snapped a little while later we found out why after going over the car with a flashlight on a lift.
