Jumped timing...Bent valves?

Recently did a head job on my evo all went good, car was running great. About two weeks after so, car would start but would idle rough. Theres a noise coming from the head, s I think i might have bent a valve or two. the timing marks are off. A new timing belt was used. What could have caused the timing to jump? What all should I replace to ensure this doesn't happen again?

Timing belt tentioner faluty or the adjustable idler loosend most likely causing the timing belt to self adjust.

The bolt/washer/lock washer that are used to tighten the pulley.

First, you should try to see what went wrong. Is the pulley loose or are you sure it is the tensioner?

Take the head off and send it in for inspection before you do anything with the new timing belt
